Why I Love “All About My Dad” Questionnaires
I don’t know about you, but the little questionnaires kids fill out are seriously the cutest. You never know what they’re going to write! Kids are so honest, sweet, and unintentionally hilarious that their answers often become treasured family memories.
Several years ago, in my son’s Sunday School class at church, his Primary teacher had the children fill out a Mother’s Day questionnaire. It was absolutely adorable! At the time, I had just designed my little elephant guest book and was working around the clock filling orders and creating new elephant-themed designs. So when the questionnaire asked, “What does Mommy do for work?” my son proudly answered, “She makes elephants!” 😂
Needless to say, I saved that one.
These printable questionnaires make such fun keepsakes because they capture a little snapshot of how children see their parents at that moment in time.

A Fun Father’s Day Activity for Kids
Ever since then, I’ve tried to make sure my kids fill out a questionnaire for Father’s Day every year. I mean, I’d happily have them do one for Mother’s Day too, but that’s usually up to someone else… hint, hint! 😉
There’s just something so special about seeing yourself through your child’s eyes. Their answers are often sweet, sometimes surprising, and occasionally downright hilarious. No matter what they write, these little questionnaires always end up being something you’ll treasure for years.
That’s why I created this free Father’s Day printable. It’s an easy and meaningful way for children to get involved in celebrating Dad. They can add their own artistic flair by coloring the page and share their childlike perspective by answering fun questions about their dad.
And of course, if the little ones are a bit too young to write, you could easily lend them a hand by asking the question and writing it for them. But be sure to keep a straight face and don’t question any of their answers! Sometimes those answers are even funnier when they’re dictated word-for-word!

There are actually two versions to choose from: one featuring a little girl and one featuring a little boy. My daughter is as girly as they come and insisted that hers have a little girl on it, so I couldn’t keep it completely generic! Both versions include the same questions and layout—the only difference is the illustration.
Our Father’s Day Printable Results
This year we’ll actually be out of town on Father’s Day, so we celebrated a little early. The day before, the kids filled out their questionnaires (with a little help from me for the younger ones) and colored them to their heart’s content.
After church, I made my husband’s favorite chocolate cake while the kids finished coloring their Father’s Day printables. When he came home from his church meetings, they surprised him with their completed questionnaires.
Apparently, my four-year-old thinks her daddy is 520 years old, and my six-year-old wrote that Daddy’s superpower is making Hondo (our dog) pee. There’s a funny story behind that one, but I’ll save it for another day!
The kids were super excited to share their answers, and we all had a good laugh reading them together. Moments like these are exactly why I love these little questionnaires so much.
